Risk Analysis and Evaluation

Finalise Hazard Mind Map
The hazard mind map is finalised in anticipation of transferring the relevant information to a mapping spreadsheet where the relationships between hazards, causes and controls can be defined.

Final Hazard Workshop/Presentation to SMEs
Feedback is given to participating SMEs regarding the hazards, causes and controls that have been captured over the course of the workshop and SME sessions, to ensure completeness.

Create Initial Hazard Log
Following on from the mapping exercise and feedback from the final hazard workshop, the initial hazard log is populated.

Script Clinical Impact Statements
As part of the initial risk scoring, consideration is given to how each hazard could create a hazardous situation, and the potential harm that could result. This information is captured on the Hazard Log as a Clinical Impact Statement for each of the identified hazards.

Initial Risk Scoring
Initial risk scoring of each hazard is completed, and justifications for the assigned scores are documented.

As part of the initial CSC information gathering exercise, evidence may have been provided for controls that are currently in place for the digital solution. These controls are taken into consideration during the initial risk scoring.

Send controls spreadsheet to programme to be populated
Following review of all current relevant controls on the Hazard Log, the commissioning programme is asked to provide evidence for any controls that are already implemented.

Create Draft CSC Report
A first draft of the CSC Report is created and continues to be populated during stages 4 and 5.

Continue to consolidate Clinical Risk Management File
Relevant documents, including control evidence, continue to be added to the CRMF throughout.

Continue to add to Controls Evidence List
Relevant documents that serve as control evidence, continue to be documented on the Controls Evidence List and added to the CRMF.
